All-on-6 / All-on-X
Our flagship treatment: a full arch of fixed teeth carried on six or more titanium implants. Maximum stability, the strongest bite, and the option that protects your bone best over time.
How it worksAll-on-6 and All-on-X full-arch implants, single implants and veneers — planned in 3D and placed by Dr. Erick Rivera in San José, Costa Rica. A fully English-speaking team, a partner hotel a short walk from the clinic, and a coordinator who answers you the moment you write.
Bisho is an implant and cosmetic practice. Most of our international patients arrive missing several teeth, wearing a denture they cannot stand, or with old work that finally failed — and leave with fixed teeth.

How it worksA full fixed arch on four implants — fewer surgical sites and a faster path for patients whose bone volume allows it. We recommend it only when it is genuinely the better plan for you.
Learn moreOne tooth, done properly: implant, abutment and a crown matched to the teeth beside it. The neighbouring teeth are left untouched.
Learn moreHand-layered porcelain veneers designed for your face, not from a catalogue — shape, shade and proportion planned before anything is prepared.
Learn moreZirconia and ceramic restorations that rebuild broken, worn or root-treated teeth — and replace old crowns that no longer seal.
Learn moreWhitening, gum contouring and cosmetic work planned together as one result — with a digital preview of the smile before we begin.
Learn moreFor the hardest cases: bite, gums, bone and aesthetics rebuilt in one sequenced plan, upper and lower, so the whole mouth works together again.
Learn moreSend photos or a panoramic X-ray from home. We tell you what is realistic, what it involves and how many days you would need here — before you book a flight.
Start onlineThese are the situations we see every week. However long it has been, and however bad you think it looks, a full-arch plan usually still works.

No teeth left in the arch

A denture that will not stay put

Loose teeth & advanced gum disease

Broken roots and worn-down teeth

Old bridges and failed work
For patients travelling from the USA and Canada we quote the arch as one package — not as a list of surprises added later.
A full arch of teeth screwed onto your own implants. Nothing comes out at night, nothing covers the roof of your mouth, and nothing needs adhesive. All-on-X simply means we use the number of implants your bone actually calls for — four, six, or more.
A full arch carried on four implants, angled to use the bone you still have. Fewer surgical sites and a shorter appointment — the right choice when bone volume and bite force allow it.
Our flagship: six or more implants spreading the load across the whole jaw. Maximum stability, the strongest bite, and better long-term protection against bone loss — which is why most of our full-arch patients end up here.
Understanding the parts makes every quote easier to read — and makes it obvious what a cheaper offer has left out.

These are his own patients — before, and after the arch was rebuilt. Nothing here is a stock photograph.

Dr. Rivera has practised dentistry in San José for 15 years and built Bisho around implant and reconstructive work — the cases other clinics call complicated. Full-arch rehabilitation with All-on-6 and All-on-X is what he does most, and what he plans personally for every patient who travels to us.
He treats patients in Spanish and English, and prefers to say plainly what a case needs — including when the honest answer is that a simpler treatment is enough.

We work with a partner hotel within walking distance of the clinic, so the day of your surgery does not end with a taxi ride across a city you do not know. Your coordinator arranges the booking with your treatment dates — you just confirm it.

It depends entirely on your case, which is exactly why we review your scan before you book anything. Once Dr. Rivera has seen it, you get the number of days in writing along with your plan — so you can book flights and time off with confidence instead of guessing.
With teeth. In most full-arch cases we place a fixed provisional prosthesis during your stay, so you leave with teeth that are screwed in place — not something you take out at night. The definitive prosthesis is fitted once the implants have integrated with the bone, usually after a few months.
We hear this constantly, and it is often out of date. A 3D CBCT scan shows the bone you actually have, and full-arch techniques are designed to use it — angled implants, longer implants, or grafting where it is genuinely needed. Send us your scan and you will get a straight answer either way.
The surgery itself is done under local anaesthesia and you should not feel pain during it; sedation is available if the idea of the appointment worries you. Afterwards expect some swelling and soreness for a few days, managed with medication — most patients describe it as far less than they had braced themselves for.
In most cases yes, provided the condition is controlled. Tell your coordinator about your medical history and any medication — particularly blood thinners or bone medication — at the virtual consultation, so Dr. Rivera can adapt the plan before you travel rather than on the day.
Yes. Dr. Rivera treats in English and Spanish, and our patient coordinators work in both languages — from your first WhatsApp message through to your follow-ups after you are home. You will never be handed a consent form you cannot read.
You keep the same WhatsApp thread you started with. Send a photo, and we will tell you whether it is normal healing, something your local dentist can handle, or something we should see — and we plan the second visit for your final prosthesis around your calendar.
Tell us what is going on and we will come back to you the same day with the next step — no cost, no obligation, and no pressure to book a flight.
Your case is with our coordinators. You will hear from us today — and if you want to send photos or an X-ray right now, WhatsApp is the fastest way.
Message us on WhatsApp, call, or send an email — whichever suits you. Someone answers around the clock, in English or Spanish.
